    Understanding Alabama's Electronic Visit Verification

    The Government of Alabama has mandated that all Medicaid-funded home care agencies implement Electronic Visit Verification (EVV).

    EVV PHASE I:

    Alabama Medicaid implemented Electronic Visit Verification (EVV) for HCBS waiver services using AuthentiCare as the sole system. The first phase launched on October 1, 2017, with no option for providers to use alternate EVV solutions.

    EVV Phase II

    Alabama Medicaid adopted an open-model EVV system on February 1, 2022, allowing providers to use the state system or submit third-party EVV data via EDI.

    Holding a governmental or third-party certification is not mandatory for caregivers who provide a range of services to the elderly. Alabama home care aides have different requirements, which depend on employment circumstances. A few of them are as follows:

    • Alabama has set standards at a nationally mandated level. The home care aide must have at least 75 hours of training.
    • A high school diploma or GED is the standard requirement. Alternatively, candidates who completed the 11th grade may qualify if they have 1 year of relevant experience, or 10th grade with 2 years of experience.
    • Home health and personal care aide jobs are expected to grow by 21% nationwide between 2023 and 2033.
    • Alabama follows the federal overtime rule—paying 1.5 times the regular hourly rate for hours worked beyond 40 in a workweek.
    • The federal and state government jointly funds Medicaid in Alabama. Medicaid's matching rate is 70 percent federal and 30 percent state. The matching rate depends on the state's per capita income and changes yearly.

    Alabama’s home care rules are flexible because the state does not license non-medical home care agencies. Instead, it emphasizes transparency—requiring disclosure of policies on training, background checks, and costs—while giving providers freedom to design services that best meet client needs.

    Time-and-a-half pay, also known as overtime pay, means earning 1.5 times your regular rate for hours worked beyond 40 in a workweek—as required under the Fair Labor Standards Act (FLSA). In Alabama, where the minimum wage is the federal floor of $7.25/hr, that translates to at least $10.88/hr for overtime hours.

    Alabama or federal law does not place any limitation on the number of hours a caregiver is permitted to work in a particular day or week. However, the employer must compensate the worker based only on the number of hours worked.

    In Alabama, there’s no legal limit on how many consecutive days a caregiver can work—state and federal law don’t mandate rest days. Employers may schedule continuous work, though federal overtime pay applies if weekly hours exceed 40.

    Alabama Medicaid has shifted to an open-model Modular Electronic Visit Verification (MEVV) system, with Alabama Medicaid serving as the state's official EVV data aggregator since February 1, 2022. Providers may use Alabama Medicaid's EVV tools or submit visits through a third-party EVV system, as long as they integrate and submit all data via Electronic Data Interchange (EDI) to Alabama Medicaid.

    The caregiver training programs can be accessed under the training tab in the CareSmartz360 portal, displaying a list of courses that have been assigned to the caregiver along with their progress.

    Activities of daily living are the general term used to describe the basic self-care routines which allow older adults to live independently. These consist of bathing & grooming, getting dressed, preparing & consuming food, and using the toilet.

    It also includes transferring between standing, sitting, reclining, and moving safely and freely.

    Under the new mandate, the EVV system must verify the following:
    • Date of service
    • Location of service
    • Individual providing service
    • Type of service
    • Individual receiving service
    • Time the service begins and ends

    EVV verifies an employee's location, hours of work for payroll and billing, reported visits are actually taking place, and that the clients are getting the care they require.
